If you prefer not to have your company name appear in the dissertation's appendices, please put a comment to this effect in the Company Name text field alongside your company name. Otherwise the questionnaire will appear in full in an appendix of the dissertation.

Any contact details you give in the questionnaire will be used only in the event that follow-up would be useful to the research. It is not anticipated that this will happen in many cases. Your contact details will not be used for any other purpose.
